School Nurse
The TMA school nurse is a registered nurse who provides health services to students and employees during the school day with the goal of maximizing academic and spiritual growth through ministering to physical needs. Student health, safety and promotion of a healthy learning environment are her primary concerns.
The school nurse is authorized to make medical referrals for illnesses or injuries that occur during school hours only. For illnesses or injuries that occur away from school, please contact your physician for medical assistance. The school nurse cannot make a medical diagnosis.
The school nurse:
- provides nursing care for injuries and illnesses which occur during school hours.
- administers physician prescribed and over the counter medications provided by parents during school hours.
- provides care for students with diabetes, asthma, severe allergies, seizures and other special health concerns. Develops Individualized Health Plans as needed.
- monitors student medical records for compliance with state law and school policy.
- serves as a liaison between students, parents, teachers, administrators and health care providers.
Medical Authorization
It is required that each TMA student have a completed and current Medical Authorization and Parent Consent Form on file before the start of each school year. Please keep the office and the school nurse informed of any changes in relevant contact information, health history or medication as they occur throughout the school year.
A parent/guardian will be contacted as soon as possible when there is an illness or injury. If a parent/guardian cannot be reached, other parties listed on the Medical Authorization Form will be contacted. If no one can be reached, necessary first aid/emergency care will be obtained for the health and well being of the student, complying with the stated parent/guardian authorization on the student’s Medical Authorization Form.
The school nurse provides care for students who are injured or become ill during the school day. School policy requires that families keep ill students at home to prevent spread of illness to classmates, faculty and staff. Students who experience any of the following symptoms within the previous 24 hours should not attend school:
Fever above 99.6
Vomiting
Diarrhea
Excessive coughing or nasal drainage
- Students who experience these symptoms during the school day will be asked to go home and should not return to school until free of these symptoms for 24 hours.
- Students experiencing communicable conditions (i.e. strep throat, chicken pox, head lice, pink eye, certain rashes, etc.) require a physician’s note delivered to the school nurse prior to re-admittance to the classroom. TMA reserves the right to require a certificate of examination.
Should a student require attention in the clinic, every effort will be made to provide care and return the student to the classroom as soon as possible. If necessary, students may rest for a brief period before a decision is made to return to class or contact parents for health related dismissal from school. Clinic contacts with students for significant injury, illness or administration of medication will also be communicated to parents via RenWeb.

Immunization Information
State of Florida immunization statutes require the governing authority of each private school to provide information to parents regarding available immunizations and vaccines.
In providing this information, The Master’s Academy neither endorses nor recommends that you immunize your child.
Current Florida law requires that prior to entry, attendance or transfer to public and non-public K-12 schools, that each child shall have on file a Florida certification of Immunization (DH680) documenting the following:
- 5 doses of diphtheria, tetanus, and pertussis vaccine ((DTaP)
- 4 doses of polio vaccine (IPV)
- 2 doses of measles, mumps and rubella vaccine (MMR)
- 3 doses of hepatitis B vaccine (Hep B)
- 2 doses of varicella vaccine (or documented case of chicken pox)
In addition, to all other compulsory school immunizations, students entering the 7th grade are required to complete the following:
- 1 dose of tetanus-diptheria-pertussis vaccine (Td or Tdap)
Every parent/guardian has the right to decide on whether or not to immunize their child. If you do not have your child immunized with the above State of Florida required immunizations, the state requires that you file a medical or religious exemption form with the school. These forms can be obtained at your county health department.
For further information about State of Florida immunization requirements, refer to www.doh.state.fl.us/disease_ctr/immune/medical.htm
This information is provided to make you aware of available vaccinations and to help you make informed decisions about the health of your child. If you have any questions or concerns about vaccinating your child, consult with your pediatrician.

Medications
A physician’s signature is required for all medications to be administered at school.
Parent/guardians may supply over the counter or prescription medications that are necessary for a student to receive during school hours.
- The first dose of any new medication may not be administered at school.
- All medication must be in the original labeled container and properly labeled with the student’s first and last name and current grade.
- Note that your pharmacist can be requested to prepare prescription medications in 2 labeled containers: one for school and one for home.
The school nurse (or a trained delegated staff member on field trips) will administer all medications . School policy prohibits students from carrying, sharing or self administering any medication with the exception of certain circumstances in which a physician provides written authorization of self care for specific chronic conditions (ie asthma, diabetes and life threatening allergies.)
All medications will be received and stored in the clinic by the school nurse . Students are not permitted to deliver medications . Parents/guardians must deliver medications in the original and properly labeled container and are expected to retrieve all medications by the last day of the school year. The school nurse will dispose of medications which have not been retrieved.
